Why Play-Based Therapy Works (and Why It Looks Like 'Just Playing')

    2 minute readBy Derek Feher

Play is a child's occupation. When therapy happens inside play, children work harder, learn faster and don't realise they're doing therapy at all.

    Key points

    • Play is a child's occupation.
    • When therapy happens inside play, children work harder, learn faster and don't realise they're doing therapy at all.

Play is the work of childhood

Occupational therapy is built around occupations — the meaningful activities that fill a person's life. For adults that's self-care, work and home life. For children, it's play. Play is where children develop motor skills, regulation, problem-solving, social skills and confidence.

So when a paediatric OT session looks like an elaborate game of obstacle courses and pretend shops, that's not therapy disguised as play — the play is the therapy. The targets are just deliberately embedded inside it.

    What's actually being targeted

Every game in a session is chosen for a reason. The obstacle course is working on motor planning, balance and bilateral coordination. The pretend café is sequencing, fine motor skills, social communication and flexible thinking. The crash mat is regulation and body awareness.

Because the child is motivated and having fun, they repeat movements far more times than they ever would in a drill — and repetition inside meaningful activity is exactly how skills consolidate.

    • Gross motor: climbing, balancing, jumping, ball skills
    • Fine motor: threading, building, drawing, manipulating tools
    • Regulation: sensory play, heavy work, calming routines
    • Social and thinking skills: turn-taking, pretend play, problem-solving

Families are part of the session

Play-based therapy doesn't end at the clinic door. A big part of my job is showing parents and carers how to weave the same targets into bath time, the playground and the lounge room — so progress happens seven days a week, not one hour a fortnight. I see children in person across the Gold Coast and surrounds, with parent coaching available by Telehealth.
