Beyond the City: Delivering Supported Independent Living Assessments in the Murraylands

Access to experienced occupational therapy services should not depend on living in a metropolitan area.

For NDIS participants across the Murraylands in South Australia, obtaining specialist assessments can involve extended waiting periods, limited local clinician availability and significant travel requirements.

These barriers can be particularly challenging when participants require comprehensive assessments to inform Supported Independent Living arrangements.

Today & Tomorrow Allied Health Services supports regional communities through scheduled outreach and telehealth services, helping participants access clinically comprehensive assessments without unnecessary geographic barriers.

Our work across regional South Australia reflects the importance of making specialist occupational therapy more accessible beyond metropolitan Adelaide.

What Is a Supported Independent Living Assessment?

A Supported Independent Living assessment examines the assistance an NDIS participant requires to complete everyday activities safely and maintain an appropriate level of independence within their home.

SIL is an NDIS support that can assist eligible participants with daily living activities and the development or maintenance of independent living skills.

An occupational therapy SIL assessment provides evidence about a participant’s functional capacity, daily routines, support requirements and disability-related risks.

The assessment may examine personal care, dressing, showering, toileting, meal preparation, household management, medication routines, mobility, communication, decision-making and community participation.

For participants with complex needs, the assessment also considers supervision requirements, overnight assistance, behavioural risks and the circumstances in which additional staffing may be necessary.

Understanding Support Needs Across an Entire Day

One of the most important components of a SIL assessment is understanding how a participant’s support requirements change throughout the day and night.

A participant might require direct physical assistance with morning personal care, prompting and supervision during household activities, and additional assistance with evening routines.

    Overnight requirements can vary considerably.

Some participants require intermittent assistance, while others have disability-related needs that require more frequent or continuous support.

A comprehensive assessment examines the nature, frequency, duration and intensity of these requirements.

This information helps establish an evidence-based understanding of the support arrangements required.

Why Regional Assessments Require Additional Consideration

Participants living in the Murraylands can experience circumstances that differ from those living in larger metropolitan centres.

Access to specialist services, public transport, community programs and support providers may be more limited.

Travel distances between essential services can also be greater.

These factors should be considered when understanding the participant’s environment and how supports can be practically delivered.

However, geographic isolation alone does not establish eligibility for SIL funding.

Recommendations must remain grounded in the participant’s disability-related functional limitations and individual support needs.

    Assessing Functional Capacity and Risk

An occupational therapy SIL assessment should examine both what the participant can currently do and the assistance required when tasks cannot be completed safely or consistently.

Relevant considerations can include falls risk, manual handling requirements, cognitive impairment, reduced safety awareness, behavioural support needs and difficulties completing essential daily activities.

The assessment should also consider the availability and sustainability of informal supports.

Where family members provide substantial assistance, it is important to understand the nature of that assistance and whether it can reasonably continue.

This allows recommendations to reflect the participant’s actual support requirements rather than assuming that family members will indefinitely provide care.

Developing a Clinically Justified Roster of Care

For participants requiring substantial daily assistance, an occupational therapy report can contribute to the development of a recommended roster of care.

This involves identifying what assistance is required, when it is required and the level of staffing necessary to deliver that assistance safely.

Recommendations should distinguish between direct physical assistance, supervision, prompting and periods where support may be required intermittently.

Where one-to-one or two-to-one assistance is recommended, the clinical reasoning should explain the specific circumstances that justify that level of support.

Overnight recommendations should similarly identify whether active assistance, intermittent responses or sleepover arrangements are appropriate.

The objective is to ensure that recommendations are individualised and supported by functional evidence.

    Why Independent Clinical Evidence Matters

SIL funding decisions require more than a statement that a participant needs continuous support.

Clinical documentation should demonstrate how disability affects daily functioning and why particular support arrangements are required.

A comprehensive assessment can provide evidence about the participant’s abilities, support needs, environmental barriers and risks associated with insufficient assistance.

This information can assist participants, families, support coordinators and the NDIA when considering appropriate living support arrangements.

    Regional Outreach and Telehealth

Today & Tomorrow Allied Health Services uses a flexible approach to regional occupational therapy.

Scheduled regional visits can be combined with telehealth consultations, stakeholder interviews and review of supporting information where clinically appropriate.

This can reduce unnecessary travel and allow important assessment components to be completed remotely.

Where direct physical observation is required, face-to-face assessment remains an important part of the process.

The method of assessment is selected according to the participant’s needs rather than convenience alone.
