Accessing specialist occupational therapy can be challenging for people living outside major metropolitan areas.

Regional, rural and remote communities often experience limited local service availability, longer waiting periods and additional travel requirements.

For NDIS participants who require specialist assessments, these barriers can delay access to important clinical evidence and support recommendations.

Fortunately, occupational therapy services can be delivered through several models, including telehealth, visiting clinicians and hybrid assessment arrangements.

Why Is Access to Occupational Therapy More Difficult in Regional Areas?

The availability of occupational therapists varies considerably across Australia.

Some communities have limited access to clinicians who specialise in complex Functional Capacity Assessments, Supported Independent Living, Specialist Disability Accommodation or assistive technology prescription.

Participants may need to travel substantial distances or wait for a visiting clinician.

For people with significant mobility limitations, fatigue, behavioural support needs or complex care arrangements, travelling to an appointment may be particularly difficult.

    Option 1: Telehealth Occupational Therapy

Telehealth allows participants to access occupational therapy services through secure video communication.

For appropriate assessments, an occupational therapist may gather information through interviews, functional observations, document reviews and discussions with relevant support people.

Telehealth may be suitable for components of:

The suitability of telehealth depends on the participant’s circumstances and whether sufficient reliable evidence can be obtained remotely.

    • Functional Capacity Assessments.
    • Daily living and support needs assessments.
    • Psychosocial functional assessments.
    • Support planning and clinical consultations.
    • Follow-up appointments.
    • Certain assistive technology reviews.

Option 2: Visiting and FIFO/DIDO Clinicians

Some assessments require direct observation, environmental measurements or physical equipment trials.

In these situations, visiting occupational therapists may provide services through fly-in/fly-out or drive-in/drive-out arrangements.

This model can be particularly useful where specialist expertise is unavailable locally.

A visiting clinician may coordinate several appointments within a region to make service delivery more practical.

Travel costs, scheduling and funding arrangements should be discussed before appointments are confirmed.

Option 3: Hybrid Occupational Therapy Assessments

Hybrid assessments combine remote and in-person service delivery.

For example, initial interviews and document reviews may occur by telehealth, followed by an in-person visit for functional observations, environmental assessment or equipment trials.

Follow-up consultations and report discussions may then be completed remotely.

This approach can reduce unnecessary travel while retaining in-person assessment where clinically required.

    The Importance of Local Collaboration

Regional assessments often benefit from collaboration with local support coordinators, therapy providers, support workers and family members.

With appropriate consent, these individuals can provide valuable information about the participant’s everyday functioning and environmental circumstances.

Local equipment suppliers and building professionals may also be important when considering assistive technology or home modifications.

Are Telehealth Assessments as Effective as In-Person Assessments?

The answer depends on what needs to be assessed.

Some functional assessments can be completed effectively through telehealth when appropriate methods and reliable supporting information are available.

Other assessments require physical examination, direct observation, accurate measurements or hands-on equipment trials.

A responsible occupational therapist should select the assessment method based on clinical suitability rather than convenience alone.
